Lost / stolen badge notifications must be made immediately once established that your badge is no longer under your control.

To report a lost or stolen badge please contact the badging office during regular office hours at (831) 333-2314 or the Non-Emergency dispatch at (831) 375-8525. 

You must also report the lost badge to your authorized signatory who is required to Complete the lost/stolen badge form via Air Badge.
